Requesting Copies of Your Medical Records
To obtain copies of your medical records from BryanLGH Medical Center you may print the authorization form  , thoroughly complete it, and either fax it to 402-481-8769 or mail to:

If you have questions or would like a copy of the authorization form mailed to you, please call the release of information section in the health information management department at 402-481-8424 Monday through Friday 8 a.m.-5 p.m.
Requested copies will be mailed to you in approximately five business days or you may pick them up at the above address at a predetermined time.
